Student Immunization Days
The State of Illinois requires all students to provide proof of receiving Measles (Rubeola), Mumps, German Measles (Rubella) and Tetanus/Diphtheria vaccines.
Failure to submit your immunization information properly will result in you being placed on restriction, which will restrict your access to University facilities and prevent future class enrollments.
Immunization forms are available online at http://scc.uchicago.edu
Students who do not have access to their immunization documentation or are missing vaccine information should attend this program.
The required measles/mumps/rubella and tetanus/diphtheria vaccinations will be available for a cost of $25/vaccine (exact cash or check).
Vaccinations are available on a walk-in first-come, first-served basis.
Students needing multiple vaccines must return for follow-up vaccinations in November.

If a language reading exam is dropped after that date, there is no refund of the $70 exam fee. In addition, if a student receives permission to DROP a course after that deadline the course title will remain on the transcript and the student will be given a grade of 'W'.
