Sarcomas of bone are one of the most challenging areas of surgical pathology (0.2% of all malignant tumors). Primary soft tissue sarcoma presenting primarily in bone (comprising 5-10% of bone sarcomas) pose a far greater challenge owing to their extreme rarity and lack of awareness. Advent of an increased number of new immunohistochemical markers and recent advances in molecular analyses has led to a deeper knowledge and a more detailed understanding of these rare bone tumors. This talk will discuss the evolution of classification of these rare bone sarcomas (non-osteosarcoma, non-chondrosarcoma and non-Ewing sarcoma) with recent updates on their current classification. This objective of this talk is to create awareness about these rare bone sarcomas, discuss their clinical presentation, diverse radiological presentations, morphologic features, characteristic immunohistochemical and molecular findings and potential diagnostic pitfalls. There is a lot of uncertainty regarding the treatment of these primary bone sarcomas with no consensus on the systemic therapies, currently available for the more well-known sarcomas. This talk summarizes the medical scientific literature and current research in the treatment of these rare sarcomas.
