We applied this approach to minimal change disease, a major cause of nephrotic syndrome. Although minimal change disease causes no detectable changes in glomeruli by light microscopy, digital spatial profiling was sensitive enough to detect two distinct subgroups of minimal change disease patients, which we can distinguish using single FFPE sections from clinical biopsies. Over the last several years, data from our lab and others have shown that macrophages promote PKD. However, these single-strand DNA changes have not been feasible to profile at single-molecule resolution. We have developed a DNA sequencing technology capable of profiling both single-strand and double-strand DNA changes with single-molecule fidelity. This includes both single-strand mismatches of canonical nucleotides and single-strand cytosine deamination damage. The revealed patterns of single-strand changes provide insight into the origins of mutations in several cancer predisposition syndromes.
