Living Well with Chronic Conditions is a six-session evidence-based workshop designed for those dealing with a chronic or limiting health condition. Each session participants learn self-management techniques and skills needed in the day to day management of any type of ongoing health condition. The program has also been proven to be effective for caregivers. On April 16th, the Leaders will hold a 30-minute information session to get things started. The six-week workshop will be held virtually on Thursday's at 12pm, from April 23rd – June 4th (no session on May 14th) and each session will be 2 hours long.
